Introduction

Avondale Chiropractic is committed to protecting your personal information when you are using our services. This privacy policy relates to our use of any personal information we collect from you via interview at the clinic or on-line. It also relates to our use of any personal information you provide to us in person, by phone, SMS, email, in letters and other correspondence.
Whenever you provide such information, we are legally obliged to use your information in line with all applicable laws concerning the protection of personal information, including the GDPR legislation.

Why we collect your data.

To ensure you receive the best possible care whilst you are a patient at Avondale Chiropractic. Information held about you may be used to help protect the health of the public and may be used within the practice for clinical audit to monitor the quality of service provided.

Personal Data we collect.

Information is collected both electronically (on our secure Cliniko (https://www.cliniko.com) patient system and on paper records.
Patient files in this clinic may include the following information:
Details about you e.g. name, address, telephone numbers, email address, date of birth.
Notes and reports about your health e.g. medical history, diagnosis and treatment plan.
Any contact the clinic has had with you e.g. appointments, clinic visits, and telephone consultations.
Relevant information from other health professionals, relatives or those who care for you.
Website contact form and online booking including name, address, telephone numbers, email address.
Text messages
Appointment reminders are sent by text and you have the right to withdraw this at any point.
Emails
Appointment reminders. From time to time you may receive an email with regards to exercises or follow ups specifically related to you plan of care. We will ask for your consent to send you these and you have the right to withdraw your consent at any time.
Clinic Telephone is a mobile phone which is password protected.

Sharing your information

We will never sell, distribute or lease your data to any third party.

How will Avondale Chiropractic use your information?

Information collected electronically and on the record card during the initial consultation will only be used by the practitioner in order to get an overall picture of your health and specific detail about your reason for attending the clinic. This will enable the practitioner to make a working diagnosis which will inform the appropriate care, including number and frequency of appointments. Your information will be updated at each appointment.

How do I give Consent?

New patients will be asked to sign in order to complete their patient record. They then sign to give consent to be examined and treated.

What if I am a patient aged under 16?

Parent / Guardian signed consent will be required if you are under 16. Children’s personal data is stored in the same way as adult data. It is only used by the practitioner to aid diagnosis and direct treatment.

When Will Avondale Chiropractic Contact me?

You may be contacted by phone, SMS or email to book and confirm appointments and for appointment reminders, agreed with your practitioner.
Occasional emails may be sent out to advise patients of changes in clinic practices or practitioner availability. There may also be the occasional newsletter which you unsubscribe from at any time. There is a general Facebook and Instagram page which will provide news and marketing.

Confidentiality

We are bound by the GCC and Data Protection Act 1998 and General Data Protection Regulation 2016 regarding sharing your information to other medical or relevant health professionals. We need your written consent before we can share records, notes or personal information.

Protecting your Data

Whilst you are receiving treatment from our clinic we will continue to securely store and use your personal data. Once you have been discharged, we will be required to retain your personal data for a minimum of 8 years.

Your rights

As we process your personal data, you have certain rights. These are a right of access, a right of rectification, a right of erasure and a right to restrict processing.
You may request a copy of your data at any time. Please make such a request in writing or by email to ann@avondalechiropractic.co.uk Please provide the following information: your name, address, telephone number, email address and details of the information you require. We will need to verify your identity so we may ask for a copy of your passport, driving license and/or recent utility bill. If you believe any of the personal data, we hold on you is inaccurate or incomplete, please contact the clinic directly and any necessary corrections to your data will be made promptly. Data Breaches

Should your personal data that we control be lost, stolen or otherwise breached, where this constitutes a high risk to your rights and freedoms, we will contact you without delay.

In the first instance please contact us in order to clarify any misunderstanding. You can contact the ICO via their website: www.ico.org.uk should you wish to make a complaint about the way we are processing your personal data.

Change of Details

It is important that you tell the person treating you if any of your details such as your name or address have changed or if any of your details such as date of birth is incorrect in order for this to be amended. You have a responsibility to inform us of any changes so our records are accurate and up to date for you.
